Misc Record: H.Q. Med Dept, 11th Va Vol Infty, Parkersburg, Va.  Dated: 13 April 1863
I hereby certify that on careful examination of Alfred Gilpin, Capt Co. K, 11th Va Infty Vols, I find a general
deranged condition of health, particularly of the nervous system, which undoubtedly had its origin in a severe
contusion received in July 1862 upon his head and situate upon the temple, producing at the time
unconsciousness for 24 hours.  Together with the symptoms of cerebral concussion after recovering from the
immediate effects of the injury he seemed to enjoy tolerable good health until exposures and fatigue, of the
service, gave evidence that his nervous system was suffering from its effect both mentally and physically and
there seems to be evidence that his difficulty is more or less dependent upon and connected with injury above
stated. In view of which , it would be expedient for him to retire from military duty as without doubt the cause
of disability will be permanent. (signed) A.D. Voorheis, Surgeon, 11th Va. Vol Infty.
